How to Become a Client: If you are a new client, please complete the New Client Application Form and include it with your sample for testing, or fax it to 859-255-1624. After your account is created UKVDL will mail you a welcome letter with your account number and other important information. If you do not have a veterinarian, please contact the UKVDL Diagnostic Services at 859-257-8283.
All samples become property of the UKVDL.

Billing Policies and Information
Monthly Billing Statements: Invoices can be sent with the final report on all non-regulatory cases. Please do not remit payment for an invoice, it will be summarized on your monthly billing statement. Payment terms are net 30, unless prepayment is required for a specific test. A monthly billing statement is sent to each client at the beginning of the month.
Overdue Balances: For clients with account balances overdue by 60 days, results for new test requests will only be released after the overdue balance is paid in full. UKVDL reserves the right to refuse testing for clients with overdue balances.
Fee Changes: Fees are subject to change without notice. Current fee schedules are available here: Test Information
Methods of Payment: Methods of payment accepted at the UKVDL are cash, checks, money orders issued in US dollars, and credit cards (Visa, MasterCard, Discover, and American Express). Please include the bottom third of the last page of your billing statement and reference your UKVDL account number on your check to ensure proper payment application.
NO payments are accepted on Sundays.
